Advance Health Care Directives

An Advance Health Care Directive is a written document that allows you to set out your written wishes in regard to health care treatment and to designate the person(s) who will make health care decisions for you when you can no longer make such decisions for yourself. In your Advance Health Care Directive, you may state your wishes regarding life-sustaining treatment, organ donation, funeral arrangements and who should serve as your conservator if you should need to have a conservator. An Advance Health Care Directive can also allow an authorized agent to access your medical information, which can be a very important provision in light of strengthened federal and state privacy laws.
